Waiver and Release

I am fully aware of all of the risks involved in participating in the event and its activities. I voluntarily agree to take on these risks.  I am solely responsible for judging my own qualifications and for my own safety during the event and while using any provided equipment, materials, or supplies. I am solely responsible for determining and assessing my health, including both my physical and mental condition. I do not suffer from any medical condition or disease that might in any way hinder or prevent me from fully participating in the event. I am qualified to use the equipment provided.

I am fully aware of all of the risks of strenuous physical activity, which could include stretching, running, lifting heavy objects (including lifting heavy objects above one’s head), anaerobic exercise, aerobic exercise as well as many other activities that I may be asked to perform while working out with the fitness providers’ coaches and trainers. I do not suffer from any medical condition or disease that may in any way hinder or prevent me from fully participating in physical activity. I have fully disclosed all pertinent medical and physical conditions to the fitness providers prior to signing this waiver. I had the opportunity to consult with my primary care physician prior to participating in this exercise program. I hereby knowingly and intentionally waive any and all claims against the fitness providers arising from or relating to my participation in activities conducted by the fitness providers.
