THIS IS A LEGALLY BINDING DOCUMENT. PLEASE READ IT CAREFULLY. BY SUBMITTING THIS FORM, YOU GIVE UP IMPORTANT LEGAL RIGHTS. 

I plan to participate in the following event hosted by Columbus Downtown Development Corporation dba Downtown Columbus, Inc. (“CDDC”):  Downtown Rhythm & Roll on June 6, 2026 (the “Event”) 

Assumption of Risk. I understand that roller skating on closed public streets is a recreational activity involving inherent and unavoidable risks. These risks include, but are not limited to, falls; loss of balance; collisions with other participants, pedestrians, spectators, volunteers, staff, barricades, cones, signs, equipment, curbs, and emergency or public-safety vehicles; skate malfunction or improper fit; uneven pavement; potholes; cracks; utility covers; manholes; drainage grates; road striping; loose gravel; debris; wet or slippery surfaces; weather conditions; and the actions or omissions of other participants or third parties. I understand that serious injury, disability, death, and property damage may result. I voluntarily assume all risks, known and unknown, arising out of or related to my participation in the Event. I further acknowledge that I have had the opportunity to inspect, or will inspect, the skates and activity area before participating, and I will not participate if I believe conditions are unsafe. 

Participant Representations. I represent that I am physically and mentally able to participate safely. I am not under the influence of alcohol, drugs, medication, or any other substance that impairs my ability to participate safely. I agree to skate within my own ability, comply with all posted rules and instructions from Event staff, volunteers, law enforcement, and route marshals, and stop participating if I feel unsafe or become unable to continue safely. I am qualified to use the equipment provided. 

Release and Waiver. In consideration for being permitted to participate in the Event, I release, waive, discharge, and covenant not to sue the Released Parties from and against any and all claims, demands, causes of action, damages, losses, costs, or expenses arising out of or related to my participation in the Event, including claims arising from the negligence of any Released Party, to the fullest extent permitted by Ohio law. “Released Parties” means CDDC, the City of Columbus, Clintonville Roller Club, David Smith, event sponsors, contractors, subcontractors, volunteers, officers, directors, employees, agents, representatives, successors, and assigns.  

Indemnification. I agree to indemnify, defend, and hold harmless CDDC from and against any third-party claims, damages, losses, costs, or expenses, including reasonable attorney fees, arising out of or related to my own acts or omissions, violation of Event rules, misuse of equipment, or injury or damage I cause to another person or property. 

This release is a binding legal document. The Released Parties will have the legal right to rely fully on all statements, agreements and promises that I make in this release. The release is binding on my executors, administrators, successors and assigns, as well as upon me.
